How do I report an injury to DAN?

Notification of an injury should be made by calling DAN Medical Information Line at 1-800-446-2671, or by contacting DAN Medical Services.

How can I get a copy of an injury form?

Copies of the current Diving Injury Report Forms can be obtained by contacting DAN Research or calling 1-800-446-2671 x237 (+1-919-684-2948 x237).

How can I obtain a copy of the DAN's annual Report on Decompression Illness, Diving Fatalities and Project Dive Exploration?

Copies of this report can be ordered online or by placing an order at 1-800-446-2671 (+1-919-684-2948). DAN Members may also download the most recent report at no cost.

How can I obtain a copy of my injury record from DAN?

A notarized Authorization for Release of Information should be mailed or faxed to the Custodian of Records in the DAN Research Department. All requests for records must be approved by the DAN legal counsel.

Does DAN Research want to know about all diving injuries?

Of course we do! DAN Research is interested in learning about every dive injury, fatality and incident, in addition to safe dives. To learn more about diving safety, we need as much data as possible. Unfortunately, DAN is not able to follow up or complete all cases we receive for many reasons. While your case is important to us, there is no guarantee it will be used in our annual report. The annual diving report includes cases that are complete, able to be followed up, and meet our inclusion criteria.
